At this time, all Parents and returning Students may log in to FinalForms (https://fairfield-oh.finalforms.com/) to complete and sign the required forms for the 21/22 school year. FinalForms saves data from season to season and year to year; however, you will need to review and sign forms every year.
Please carefully review your information, update if necessary, and sign all forms.
Only students in grades 6-12 will be required to sign any forms. If you are a newly enrolled student for the 21/22 school year, you will be unable to sign forms until the school year begins and you have accessed your Fairfield-assigned email account.
A few notes about forms:
- The Babysitter/Daycare Transportation form will be electronic and available on Final Forms for the 21/22 school year. Alternate addresses only need to be provided if you are requesting school bus transportation to or from an address other than your residence. If your child is riding to or from school on a daycare bus/van, you do not need to provide an alternate address.
- The Free/Reduced Lunch and fee waiver form will be available on July 1.
- Parking Pass applications for high school students will be available on August 2.
- Additional forms may be added to your student’s Final Forms account throughout the summer, or even into the school year. Please be sure to check your Final Forms account occasionally to be sure all forms have been signed and all contact information is up to date.
