Playing a sport next year? Thinking about it? Join the Fairfield City School District and Mercy Health Monday, May 18, 2015 and get your physical done that day!
$10.00 is the cost! A free T-Shirt is the prize!
Join us during the following times:
Seniors: 5:45PM
Juniors: 6:15PM
Sophomores: 6:45PM
Freshman: 7:15PM
Middle School: 7:45PM
Enter Door #9 at the back of the High School.
In addition, Fairfield City School’s Athletic Department wants this to be even easier so we have adopted a paperless form system. All but the physical itself can be filled out and submitted online. Go to https://fairfield-oh.finalforms.com/ to register your child(ren) and fill out the required forms. It will walk you through it step by step.
Please know that once you submit all forms online through Final Forms, you will only be required to update and sign forms once per school year online. Understand that the online update will be a few minutes at most! The physical will still be required yearly.
